Questionnaires
A questionnaire is a simple form for feedback or votes: "What should we add next?", "How was the last event?", "Which car pack do you want?". It uses the same question types as an application form, but there is nothing to review: no staff queue, no Discord post, no roles handed out. Players answer once and you read the results.
Questionnaires live on the Applications page, so they need a plan that includes applications (see Plans). Only people with Applications set to Manage can create them, see the Questionnaires tab, and read the results.
Create a questionnaire
- Go to Applications → Questionnaires and press New questionnaire.
- Type a Name.
- Decide on Anonymous responses (see below). You cannot change this after publishing.
- Press Create questionnaire.
On the Questions & settings tab, fill in the Name, an optional Description, and your questions. The question types are the same as on application forms: see Questions. Pick one and Pick several work well for votes. Press Save draft to keep your work without making it public.

Players still sign in for anonymous questionnaires, but only so each account can answer once. The answers are stored without their identity.
In an anonymous questionnaire, avoid questions that make players identify themselves, like "What is your character's name?". The questionnaire cannot hide what they type into the answer.
Publish and open
Press Publish questionnaire. The dashboard asks Publish this questionnaire?; press Publish and open. Players can respond right away.
After publishing, you cannot change the questions or switch between named and anonymous. That way everyone answers the same questionnaire. To ask something different, create a new questionnaire.
Once it is published you have one button at the top:
- Close responses stops new responses. The questionnaire stays on your portal page, marked as closed.
- Reopen responses lets players answer again.
Share it
The Share tab has the questionnaire's link, in the form https://apply.qbox.re/<your-slug>/questionnaires/<questionnaire-address>. Post it in your Discord.
Players can also find it on your portal page under Have your say, with whether it is anonymous or named.
What players see
- They open the link and see whether their response is anonymous or shows their username.
- They press Sign in with Discord to respond.
- They answer and press Submit response. The page reminds them: "Your response is final once submitted."
Each Discord account can respond once. Players never see the results.
Results
The Results tab shows how many responses came in, a tally for every Pick one and Pick several question, and every response below it, with the player's username for named questionnaires. Press Refresh to load new responses.
Results are only visible in the dashboard, to people with Applications set to Manage. Nothing about the results is shown on the portal.
Troubleshooting
Publish questionnaire is greyed out. The questionnaire has no questions players can answer. Section headings alone are not enough.
A player gets "You have already responded to this questionnaire". Each Discord account can respond once, and responses cannot be changed after they are sent.
Players see "This questionnaire is closed. It is no longer accepting responses." You pressed Close responses. Press Reopen responses to accept answers again.
Players see "Questionnaire not found". It has not been published yet, or the link is wrong. Copy the link again from the Share tab.
